of legal age, I hereby AUTHORIZE AND CERTIFY:

1. I have been informed of and acknowledge my participation in the activity(ies) conducted by Centro Esperanza, Inc., under the Heritage Tourism and Community Service Program.

2. That I agree to participate in the training and education activities, study trips and tours organized by the Centro Esperanza, Inc. in the Colobó community of the town of Loíza as well as in any other place where activities, meetings or tour services are held.

3. I acknowledge, certify and agree that the activities to be performed with Centro Esperanza, Inc. and/or collaborators may require physical activity that may vary from easy to moderate; I have been duly informed about the requirements, the appropriate clothing to participate in this activity, and about the environmental conditions and equipment to which I may be exposed. I confirm that I understand the requirements and necessary conditions.

4. I acknowledge, certify and accept that, as in any activity, in the practice and participation, accidents may occur that cause some kind of physical damage or injury to the participant due to natural causes or as a result of the actions of others.

5. I acknowledge, certify and agree that I have the emotional condition and physical capacity required to perform all required activities: exposure to the sun, rain, walking distances on uneven terrain, or other elements related to activities performed in nature or facilities of Centro Esperanza, Inc. and/or collaborators.

6. I commit myself to review, promote and follow the safety rules, and rules of conduct and healthy coexistence duly explained by the personnel in charge.

7. I acknowledge and agree that I will be responsible for any act of aggression or any felony I commit against the directors, co-directors, professionals, employees, volunteers, properties, collaborators, facilities and/or any entity that supports, is affiliated or allied to Centro Esperanza, Inc..

8. I forever RELEASE and hold harmless the following from liability Centro Esperanza, Inc. , The Company, its Executive Directors, the members of the Board of Directors and all its employees, facilitators, officers and volunteers, for any damage or harm, direct or indirect, that may be suffered while participating in the activities, meetings, workshops or any other activity. This release includes the period while waiting for the activity to begin or at the end of the activity and also during the time of travel to and from the activity. This release applies to the result of any action or omission by me or any member of my family or companion. P.O. 9. Pay and/or reimburse any expenses, bills of collection, judgments, rulings or orders rendered by any court against the Centro Esperanza, Inc. as a result of any act or omission of mine, I will also pay any costs related to repairs, replacement of equipment or materials, or improvements that may have to be made because of the acts or omissions.

10. I authorize representatives of the Centro Esperanza, Inc., The Company, its teams, agents or any officer to arrange for any emergency medical service required for an emergency situation in which it is in danger, without this representing any payment for the costs of medical service by the Company, or any of its employees, agents or any officer to arrange for any emergency medical service required for an emergency situation in which it is in danger, without this representing any payment for the costs of medical service by the Company. Centro Esperanza, Inc. Payment and/or reimbursement of any medical expenses or bills will be my responsibility.

11. I AUTHORIZE to Centro Esperanza, Inc.. to use, for an indefinite period of time, my image captured during the activities carried out, whether photos, videos or any audiovisual material, to be used in promotional materials, in fundraising campaigns or in any communications material carried out by the Centro Esperanza, Inc.

12. I AUTHORIZE to Centro Esperanza, Inc. to use, for an indefinite period of time, my image captured during the activities carried out, whether photos, videos or any audiovisual material, to be used in promotional materials, in fundraising campaigns or in any communications material carried out by the Centro Esperanza, Inc.

13. I waive any right, claim or interest I may have to control the use of my identity; I understand and agree that I will not receive financial or other remuneration for audiovisual material that is published in the media that I use, and I agree that I will not receive financial or other remuneration for the audiovisual material that is published in the media. Centro Esperanza, Inc. decide to use my image, once or multiple times as described above and as you deem necessary.
